In my 15 years experience in the web business, most website visitors look for a “Contact” page of some sort. (also you want website visitors to contact you right?) The more ways a website visitor can contact you on the site, the better. Also, its very important for search engine optimization purposes that you have a page that has your address, city and state on it. This is for LOCAL search engine optimization, so Google and other search engines will note your site as being at a certain geographical LOCATION and may rank it higher with local keyword searches. We also suggest that for Search Engine Optimization purposes that you add your address, and phone number etc at the bottom of each page of the site.
